How do you know if your child has ADHD?

My son is 4 and he I don't know if he is hyperactive or has ADHD? He runs constantly and cannot sit still, even having problems with this in school. His speech is behind due to not listening. He has a hard time napping and sleeping a full amount of time. Believe me I am not trying to "label" my son and am very anti-med at his age but if he needs treatment to learn I will take him in.
